Senate Bill 658, Section 1(E) became effective July 1, 2021.

"For school enrollment, a parent or guardian shall provide one of the following:

  1. Current, up-to-date immunization records; or
  2. A completed and signed exemption form.

We are required by law to uphold this new requirement.  We are giving parents/guardians 30 days from the date you are notified to obtain the required immunization(s).  We understand that with the COVID-19 pandemic, it has become difficult to schedule appointments for shots.  If you can't get your child in within the 30 days, you must send us the time and date of your child's appointment. 

Attached, you will find a certificate of exemption form should you choose to opt out of immunizations.  This form has to be returned to Mrs. Shannon to be mailed to the Oklahoma State Department of Health.  (It won't be accepted if you mail it.)  There is also a Guide to Immunizations form that outlines what immunizations are due each year.  If you have questions, please reach out to me.
